Main Target

Animal studies have identified signal transduction pathways within the heart that render it very resistant to infarction when they are activated.  While a number of drugs were identified which could activate the pathway it was thought that activation had to occur prior to the onset of ischemia.   That made the approach impractical for patients presenting with acute myocardial infarction since pretreatment is impossible in that setting.   Several years ago it was discovered that preconditioning actually exerts its protection in the first minutes of reperfusion.   Furthermore it was found that those pathways could still be activated to confer the preconditioning-like protection right up to the moment of reperfusion.  As the list of strategies for activating the preconditioning program at the time of reperfusion grows, several of these have inevitably found their way into clinical trials.  This year’s meeting focuses on recent clinical trials with cardioprotective agents in patients with congestive heart failure and myocardial infarction.  Foremost among these are natriuretic peptides and ischemic pre- and post-conditioning.  In addition we will take an in-depth look at the challenges that confront the clinical investigator attempting to translate preclinical data into a clinical trial design.
